I don't know if this helps, but it may caused by concurrent lxd/lxc 
operations.
I can see there a bunch of lxc processes (cron jobs) and I assume that 
upgrade happened (trigged by puppet) when these processes also were running.
I also can see lxd in this state:

12465 ?        Ssl    0:00 /usr/bin/lxd waitready --timeout=600
